Why Your Lipid Profile Has So Many Numbers
A standard lipid profile blood test does not measure just one thing called "cholesterol." It measures several different lipoproteins: proteins that carry cholesterol and fats through the bloodstream. Each has a different job, a different risk profile, and responds differently to diet, lifestyle, and medication.
After 50, the balance between these numbers shifts in ways influenced by hormonal changes, reduced physical activity, dietary patterns, and the cumulative effects of metabolic ageing. Understanding what each number means is the first step to an informed conversation with your doctor about cardiovascular risk.
Total Cholesterol: The Least Useful Number in Isolation
Total cholesterol is the sum of all cholesterol-carrying particles in your blood, including LDL, HDL, VLDL, and other lipoproteins. It is the first figure listed on most reports.
The problem with total cholesterol as a standalone measure: it tells you very little without knowing its components. Someone can have a high total cholesterol but excellent cardiovascular risk if most of that cholesterol is HDL. Conversely, a normal total cholesterol can mask a dangerous LDL-to-HDL ratio.

LDL: Low-Density Lipoprotein
LDL is often called "bad cholesterol," though the label is an oversimplification. LDL carries cholesterol from the liver to cells throughout the body, which is a necessary biological function. The problem arises when LDL levels are chronically elevated.
At high concentrations, LDL particles penetrate arterial walls and become oxidised. This oxidised LDL triggers an inflammatory response. Immune cells (macrophages) engulf the oxidised LDL and become foam cells, which accumulate in arterial walls and form plaques. Over time, these plaques narrow arteries and increase the risk of heart attack and stroke. This is the mechanism behind atherosclerosis.
What is optimal LDL? For most healthy adults over 50 in India, below 100 mg/dL is generally considered optimal. For those with existing cardiovascular disease, diabetes, or multiple risk factors, below 70 mg/dL may be the target. Your cardiologist will specify your personal target based on your overall risk profile.
HDL: High-Density Lipoprotein
HDL is commonly called "good cholesterol" because it performs what is known as reverse cholesterol transport. HDL particles travel through the bloodstream, collecting excess cholesterol from peripheral tissues and arterial walls, and transporting it back to the liver for processing and excretion.
Higher HDL levels are consistently associated with lower cardiovascular risk. An HDL below 40 mg/dL in men and below 50 mg/dL in women is considered a risk factor. An HDL above 60 mg/dL is considered cardioprotective.
HDL <40
HDL below 40 mg/dL is classified as a cardiovascular risk factor, independent of total cholesterol or LDL. In India, low HDL is more prevalent than high LDL as the primary lipid abnormality in urban adults.
After 50, HDL levels tend to decline in women as oestrogen decreases (oestrogen supports HDL production) and can fall in men with central obesity and insulin resistance. This makes HDL support a particularly relevant concern in midlife cardiovascular management.
VLDL: Very Low-Density Lipoprotein
VLDL is the least discussed lipoprotein but plays an important role in lipid metabolism. The liver produces VLDL to export triglycerides (fats derived from dietary carbohydrates and excess calories) into the bloodstream for delivery to cells.
As VLDL circulates, it loses triglycerides to cells and is eventually converted into LDL. Persistently high VLDL therefore feeds into higher LDL over time.
VLDL is not directly measured in a standard lipid test. It is estimated as approximately one-fifth of your triglyceride level. A triglyceride reading above 150 mg/dL usually correlates with elevated VLDL. The reference range for VLDL is typically 2 to 30 mg/dL.
Triglycerides: The Fourth Number That Matters
Triglycerides are fats circulating in the blood. They come from calories consumed but not immediately used, particularly from refined carbohydrates, sugars, and alcohol, which the liver converts into triglycerides for storage. Elevated triglycerides are an independent cardiovascular risk factor and are closely associated with low HDL, high VLDL, and insulin resistance.

- Reduced physical activity lowers lipoprotein lipase activity, reducing triglyceride clearance from circulation.
- Increased oxidative stress accelerates LDL oxidation, raising the atherogenic potential of circulating LDL particles.
Understanding your lipid panel is not about memorising numbers. It is about knowing which numbers to ask your doctor about, and why each one tells a different part of your cardiovascular story.
